Life Without Barriers offers a range of programs designed to support individuals experiencing mental health distress and challenges in their daily lives. Our services are tailored to meet the diverse needs of our community, providing both intensive crisis support and non-acute psychosocial support. Intensive Crisis Support

This program offers recovery-oriented rehabilitation and support for individuals aged 16-64 with mental health conditions requiring intensive short-term assistance. Services are provided one-on-one at home and in the community. This program will also support people who have been unable to engage and start utilizing their existing services including the National Disability Insurance Scheme (NDIS) or My Aged Care (MAC). We will support people to successfully transition to their NDIS or MAC plans.

Eligibility

  • Ages 16-64.

  • Exceptions for 65+ if NDIS or MAC services are not accessible.

Accessing services

To access this service, SA Health mental health referrers such as your GP, inpatient services, Psychiatrist, alongside the person, must complete an application. Once approved, documents are forwarded to an NGO provider, and a peer or psychosocial support worker is assigned. The support worker aims to contact the referrer within 24 hours.

Service provision

  • Available 7 days a week, 8am-8pm (flexible based on needs).

  • Support available for up to 6 weeks.

Who Can Benefit from Intensive Crisis Support?

Individuals experiencing Mental health challenges needing intensive support to manage distress, improve safety, and establish long-term community connections. Suitable for those at risk of requiring acute care or recently discharged from an acute setting.

  • Accessing social, educational, and recreational activities.

  • Using public transport.

  • Developing a healthier lifestyle.

Developing the Support Plan

An Assessment of Need guides the Individual Support Plan (ISP), detailing tasks, timelines, visit frequency, and service hours. Review meetings at the midway point ensure progress and address any needed changes.

Privacy

A confidentiality agreement is required to allow information sharing between referrers, NGO providers, and relevant agencies. All service providers adhere to strict confidentiality standards.

Non-Acute Psychosocial Support

This program offers recovery-oriented rehabilitation and support for individuals aged 16-64 experiencing mental health challenges and distress. Services are one-on-one at home and in the community, tailored to the individual's changing needs, without duplicating existing services like NDIS or My Aged Care (MAC).

Eligibility

  • Ages 16-64.

  • Exceptions for 65+ if NDIS or MAC services are not accessible.

Accessing Services

SA Health referrers, alongside the individual, complete an application. Approved documents are forwarded to an NGO provider, and a peer or psychosocial support worker is assigned, contacting the individual within 48 hours.

Service Provision

  • Available 5 days a week, 9am-5pm (flexible based on needs).

  • Support provided for up to 4 months, extendable to 12 months in exceptional circumstances.

  • Services available in specified Local Health Networks.

Who Can Benefit from Non-Acute Psychosocial Support?

Individuals experiencing mental health distress and challenges needing non-acute psychosocial support to enhance wellbeing, functioning, and community connections.

  • Accessing suitable housing.

  • Assistance with accommodation and tenancy.

  • Developing daily living skills

  • Accessing social, educational, and recreational activities.

  • Using public transport.

  • Developing a healthier lifestyle.

  • Reconnecting families wherever possible

  • Determining access to NDIS or MAC.

How to get a referral

Referrals to these programs can be made by your General Practitioner (GP), Psychiatrist, clinical supports, inpatient units within hospitals, and soon, the Urgent Mental Health Care Centre.

Wellness Connect

Life Without Barriers is part of the consortium delivering Wellness Connect (Formerly NPSP National Psychosocial Support Program) across the Adelaide Metro region, led by Neami National. All referrals must go through Neami:

This is the only program in Adelaide that accepts self-referrals. We provide non-clinical support using recovery-oriented practice skills. You lead your support, and we walk alongside you, always listening, collaborating, and advocating with your consent.

Adelaide Metro Area

Wellness Connect services are for individuals experiencing mental health distress and challenges who experience reduced functional capacity and are unable to access NDIS supports. The service aims to:

  • Improve access to psychosocial support services.

  • Enhance mental health outcomes and service equity.

  • Reduce the need for intense and acute health services.

  • Promote optimal use of the health system.

Wellness Connect is funded by the Adelaide Primary Health Network (PHN) and delivered through a consortium model across metropolitan Adelaide. Led by Neami National, the consortium includes Life Without Barriers, Mind Australia (Mind), Skylight Mental Health (Skylight), and Community Access and Services SA (CAaSSA). Wellness Connect provides low-intensity, community-based, one-to-one support and group activities to support recovery for people with mental health challenges not receiving NDIS support.

Recovery-Oriented Practice

Recovery-oriented practice recognises that you are the expert in your own life. We walk alongside you, providing support and coaching to help you achieve your goals. These goals are agreed upon during our initial meetings. We make referrals to community supports that interest you and meet your needs. Our approach involves listening, collaborating, and advocating with your consent.

We discuss what has worked well for you in the past and what hasn’t, allowing us to build a personalised plan. This helps you remain independent and live well in your community. Our support is non-clinical, but we collaborate with your clinical supports with your consent.
